About Us

The BAYOU Bridges home visit program aims to bridge the gap in mental and behavioral health care caused by the current shortage of mental and behavioral health specialists. Our teams of primary care physicians are trained to screen for, diagnose, and treat common mental and behavioral health conditions, and the home visit setting allows the team to provide you with individualized, contexualized care. Whether you're managing a chronic condition or need help with a new mental health concern, our team is here to support you.
