Excused Absence Form


Use the above link to access the Excused Absence form.  If you have any questions regarding the policy regarding an excused absence, please refer to the link at the top of the page once you click on the form link.

 

Official Transcript Request (current TUSOM medical students only)

Document Request (Good Standing Letters, Clerkship Letters, Proof of Malpractice Notice/Certificate, Jury Duty Dismissal Letter)

For any other letter not listed, you can request it in the "other" field.



For ACLS Certification Letter, you received proof of certification when you completed ACLS/BLS training. Use this card to document your training. For questions about your training or card, please contact Tulane’s Sim Center.





Tulane University School of Medicine does not provide background checks, those can be done through NOPD.

Annual Required Paperwork

Bloodborne Pathogens 

Required for T2s, T3s & T4s

  • Bloodborne Pathogens Training is completed in Canvas. It can be accessed via here.
  • The BBP training must be completed between the hours of 8:30 a.m. - 4:30 p.m., M-F, and not during university closures.
  • For any questions or concerns regarding this training, contact Ashley Cassell at acassell@tulane.edu or (504) 314-2735.
  • Be sure to keep a copy for your records. You will need this when you apply for away rotations.
 

Financial Responsibility, Honor Code, Technical Standards Attestation

Required for all students. Once completed, should be emailed to saforms@wave.tulane.edu

HIPAA Presentation and  Quiz

Required for T3s, due by T3 Orientation

  • Completed via Canvas. Students will receive an email when assigned.
  • Be sure to keep a copy for your records. You will need this when you apply for away rotations.
