A former player's health is the starting point for a successful transition to life after football. The Trust Brain and Body Assessment Program is designed to assess the health needs and to provide world-class medical care to former NFL players. The Trust has assembled a team of highly regarded medical centers, and each has direct experience in treating NFL players and is committed to helping them develop and maintain a healthy brain and body. Each partner will also provide a recommendation containing a plan of action that each player can follow in his home community. Finally, the partners will follow up with the players via phone calls, emails, and telemedicine mediums to continue to encourage and advise players with regard to their recommendations.

Our interdisciplinary treatment team at Tulane Institute of Sports Medicine (TISM) takes pride in our services to support Players' health and safety both on and off the field. The Brain and Body Health Assessment at TISM provides Players with facilitated access to comprehensive evaluation and treatment plans. Those plans target and reduce potential health risks, promote overall wellness and determine Players' potential health problems. If Players wish to include their significant other and/or family members, our team is available to provide education concerning the medical condition(s) that are identified by the evaluations and the treatment(s) that we recommend.
Overview
Each Player participating in the Brain and Body Health Assessment at TISM is assigned a care manager who will ensure rapid access to medical care and facilitate communication, and will coordinate prompt care. Following the initial evaluation, the Player will receive an individualized treatment plan with resources to access the recommended treatment. The Player will also be provided follow-up contact after the initial evaluation, along with copies of the evaluation results and treatment plans.

Start by contacting The Trust and request your Brain and Body Assessment. Once you are referred to Tulane, our care coordinators will contact you to schedule your visit. Before you arrive to Tulane for your assessment, please print and complete the Brain and Body Assessment Form. Please bring the completed forms with you to your first appointment at Tulane Institute of Sports Medicine.
