About Your Visit
If you typically wear contact lenses, please bring a case to put them in during your eye exam, or wear your prescription glasses.
Please bring an updated list of your medications including:
- Medication name
- Medication dose in milligrams
- How often you take them.
- Any other medical records or health information documenting major medical eye evaluations or surgery.
