Orientation Checklist

Listed below are requirements that must be completed prior to starting your first clinical rotation on May 7, 2018.
  • HCA Training

  • Respirator Fit Testing
    N95 Questionnaire must be completed before testing and uploaded onto the Patient Portal. Click here for more information.

  • Student Health Drug Screening and PPD Testing

  • Bloodborne Pathogens Training|
    Completed via Training Wave. Students will receive an email when assigned. The course can also be accessed by clicking here

  • HIPAA Training and Quiz

  • Empire/EPIC UMC Training (online modules in preparation for in-person training on the first day of orientation)

  • VA Training, Fingerprinting, and Photo Session (Completed in Jan/Feb. If you have not completed it, please contact 

  • Confirm registration completed via Gibson

  • Sign Financial Responsibility/Handbook/Honor Pledge Form

Important Reminders from the Health Center


Information on TB Screening, Urine Drug Screening, and Respirator Fit Testing

N95 Respirator Medical Evaluation Questionnaire--Must be completed before respiratory fit testing. Click here for information. 

Please upload the completed questionnaire onto the Patient Portal, accessible at, fax to the Downtown Health Center at 504-988-3217, or drop it off at the Downtown Health Center.



Student Health will be placing PPD skin tests in Room 220 in Murphy for the rising T3 class.

PLEASE NOTE: Even if you have had a positive PPD, if you get the Quantiferon gold blood test instead of a PPD, have taken INH, etc., please stop by and complete a TB Questionnaire. 


You can disregard any appointment reminders you receive regarding the urine drug screen:

  1. The urine drug screens will be in Room 220 in the Murphy Building (not the Downtown Health Center).
  2. Please disregard the lab appointment time
  3. You will not be charged any late/no show fees
  4. Please do not cancel your appointment online or by phone


Since saccharin aerosol is used during the fit testing, food or drinks with artificial sweeteners should NOT be consumed within 15 minutes prior to the scheduled test.

In order to be fit tested, males are allowed to have a mustache but NOT a beard, goatee, or long side burns. In accordance with the OSHA Respiratory Protection Standard, individuals with facial hairs that interfere with the respirator seal CAN NOT be fit tested.
