Bring any prior records and diagnostic studies (such as x-ray, MRI, CT-scan) with reports relating to your condition.
Medicaid referrals should be made out to Tulane orthopedics and are required at the time of your appointment booking.
School insurance forms must be filled out completely and signed by a school official in order to be seen.
